Humana, Interwell Health Expand Kidney Care Partnership to Florida

Humana Inc, a leading health and well-being company, has announced the expansion of its value-based care agreement with Interwell Health, a kidney care management organization. 

The extension will now offer eligible Humana Medicare Advantage members in Florida access to Interwell’s care services for chronic kidney disease (CKD) and end-stage kidney disease (ESKD). 

This move follows their established partnership across other U.S. states, reflecting a growing commitment to improving outcomes for kidney disease patients.

The agreement allows Medicare Advantage members in Florida to benefit from Interwell’s comprehensive kidney care services, which include access to a network of 2,000 nephrologists, renal care coordinators, and multidisciplinary support teams. 

These services are designed to provide coordinated care both in-person and virtually, helping patients manage their conditions more effectively.

Value-Based Care for Kidney Disease

The value-based care model, which focuses on improving patient outcomes through coordinated efforts, is at the heart of this expanded agreement. 

Under this model, Interwell Health will offer eligible members access to its nephrologists and virtual support services. The aim is to provide a more holistic approach to treating kidney disease, particularly in chronic and end-stage conditions.

Humana’s value-based care efforts have already shown positive results in other regions. Notably, the company recorded a 5% reduction in unnecessary hospital admissions in 2023 through similar partnerships. 

One notable patient story involved Mary Johnson, a beneficiary of the program, who was able to undergo a pre-emptive kidney transplant, facilitated by Interwell’s care team.

“This expansion allows us to further reach individuals with CKD and ESKD, ensuring that they receive the care and support they need. We know that patient outcomes improve when there is a dedicated team that supports their unique healthcare needs. By expanding into Florida, we are continuing our mission to provide value-based care that truly makes a difference,” said Dr George Hart, Chief Medical Officer of Interwell Health. 

Expanded Reach for Specialized Kidney Care

With this latest expansion, Interwell Health’s services are now available in 14 states for members with chronic kidney disease and 39 states for those with end-stage kidney disease. 

This geographical spread ensures that more patients across the US have access to the specialized care necessary to manage these severe health conditions.

The expansion is significant, especially as kidney disease is a leading cause of health complications in the US, affecting millions of people. 

Providing access to specialized care through partnerships like this is crucial for improving both the quality of life and clinical outcomes for those affected by kidney conditions.
