The collaboration seeks to create a post-acute care technology platform for supporting intelligent patient care within the home-based care industry.

This partnership aims to address a prevalent issue for post-acute care providers - a lack of communication and coordination. When patients transition from hospitals or nursing homes to their homes, they often leave without a proper care plan.

Medalogix and Forcura aim to address these challenges through their combined expertise. The companies will leverage data-driven solutions to improve patient outcomes and drive better business performance.

The merger will also facilitate sharing additional data with upstream providers referring patients. Moreover, it aims to simplify the referral-to-admissions process for home-based care providers.

Under the agreement, Berkshire Partners will own the majority of the combined entity, while The Vistria Group will hold a minority stake.
