Elevance Health Partners with CD&R for National Primary Care Initiative

This platform, combining Apree Health and Millennium Physician Group, is a significant development in the healthcare sector, integrating resources and improving patient care coordination.

Elevance Health, formerly known as Anthem, in collaboration with private equity firm Clayton, Dubilier & Rice (CD&R), launched a new care delivery platform, Mosaic Health. 

The platform, a fusion of apree health and Millennium Physician Group aims at integrating resources and enhancing care coordination for diverse patient populations across the United States.

Mosaic Health is designed to operate across various healthcare markets, including commercial, individual, Medicaid, and Medicare. The platform’s reach is extensive, covering nearly one million patients across 19 states through its integration of apree and Millennium. 

This strategic collaboration was initially announced in April, with the recent launch signaling a critical step towards enhancing patient care through a combined digital and community-based care model.

“Mosaic Health will innovate on existing risk-based care delivery models. I am excited to work closely with its operating companies and Elevance Health to foster collaboration and help Mosaic Health’s operating companies better serve our providers and deliver exceptional care and services to more patients and communities,” said CD&R Operating Partner Clay Richards, who has been appointed as Mosaic Health’s executive chairman.

The Partnership Goals

Mosaic Health aims to streamline operations by allowing its constituent companies to share resources and information more effectively. 

This integration is expected to push the platform towards a whole-person approach to healthcare, enabling better care coordination and patient engagement. 

Despite the consolidation under Mosaic Health, both apree and Millennium will retain their existing brands, operations, and locations.

Elevance Health plans to enhance Mosaic’s offerings further by integrating its Carelon Health unit, pending regulatory approvals.

The integration of apree and Millennium within Mosaic Health is also expected to bolster a community-based care model, reinforced by digital solutions for patient engagement and care navigation. 

This approach aims to enhance accessibility and affordability in healthcare, aligning with the broader goals of Elevance Health and CD&R.

“Mosaic Health demonstrates CD&R’s continued commitment to investing in and growing innovative healthcare companies that increase access to high-quality and affordable healthcare, and we are excited for this new chapter of innovation, expanded access and growth,” Richards added.

The platform’s operations will span multiple healthcare markets, with a focus on improving care delivery models that cater to the unique needs of diverse patient populations.
